To qualify as a domestic student, and to be entitled to the Government tuition subsidy, you must be:

• A citizen of New Zealand (including students from the Cook Islands, Tokelau, or Niue who have New Zealand citizenship) or a permanent resident of New Zealand or a citizen or permanent resident of Australia residing in New Zealand

You must provide evidence of citizenship or permanent residency.

To do so you must produce one of the following:

• Birth certificate with place of birth stated as New Zealand, Cook Islands, Tokelau, or Niue. A New Zealand Passport. A certificate of identity. A statement of Whakapapa, including date of birth, countersigned by a Kaumatua. A New Zealand certificate of citizenship. Overseas passport with residency stamp Please note that your name, date of birth and residency as entered on this enrolment form will be included in the National Student Index and will be used in an Authorised Information Matching programmes with official government information such as the New Zealand Birth Register, to verify the information provided.

In signing this enrolment form you authorise the disclosure of your personal information on the understanding that Front-Line Training Consultancy will comply with the legal requirements in relation to the use and disclosure of personal information, as set out in the Privacy Act 2020. The Education and Training Act 2020 and other relevant legislation.

The information sought in this personal record form is subject to the provisions of the Privacy Act 2020 and will be held by Front-Line Training Consultancy Limited on your electronic file. It is required for the following purposes:

• To ensure you are supported to your personal ability All client information is stored in a secured electronic file that is relevant to the individual and only accessible from designated authorised Front-Line Training Consultancy employees only The information collected, received and held by Front-Line Training Consultancy may be accessed by you at any time on request. You may also request correction of the information
• To determine if Front-Line Training Consultancy needs to provide any special services, support or facilities to assist you
• All information will be held for the duration of Front-Line Training Consultancy requiring to hold enrolment details of individuals In the event information will be disposed of, Management will engage with their technicians to ensure that all information is deleted from the system, in a secure manner Handwritten forms will be scanned and saved into each individual electronic file, originals will be shredded immediately
• To select learners for programmes and check evidence for eligibility Supply of information to government agencies and other organisations Front-Line Training Consultancy supplies data collected on this form to government agencies, including: Ministry of Education, Education New Zealand, The New Zealand Qualifications Authority, The Tertiary Education Commission, The Ministry of Social Development and Inland Re venue . Those agencies use the data collected for statistical, funding returns, academic outcomes and confirmation. When required by law, Front-Line Training Consultancy releases information to government agencies such as the New Zealand Police, Department of Justice, Ministry of Social Development, and the Accident Compensation Corporation (ACC).
